The calcium-to-phosphorus ratio is a central dietary metric in Ray Peat's framework, where the ratio itself matters more than the absolute intake of either mineral within normal ranges. Peat argued that a chronic imbalance favoring phosphorus is a major stimulus to the parathyroid hormone (PTH), which in turn drives the body to pull calcium from bones and can promote calcification of soft tissues and blood vessels. He identified that many people habitually consume a ratio of 5, 6, or 7 times as much phosphate as calcium, a pattern he linked to increased risk for serious problems, including lung cancer.

The primary dietary sources of excessive phosphorus are meat, seeds, beans, nuts, and whole grains, with the germ of grains containing most of the phosphate. Peat noted that meat can have a phosphate-to-calcium ratio as high as 10 to 1, while refined grains like white rice and white flour, having the germ removed, present a lower phosphate burden. In contrast, the vegetable source of calcium with the least phosphate is leaves, and because cows naturally prefer to eat leaves, cow's milk provides a very good ratio of roughly 1.3 to 1 in favor of calcium. Peat considered a milk and cheese-centered diet the most practical way to maintain a safe ratio, recommending that phosphate intake probably should not exceed about 2 parts for each part of calcium.

Peat described the long-range optimum ratio as somewhere between 1 to 1 and up to 2 to 1 in favor of calcium, a range he considered very safe. He stated that a quart of milk provides about 1200-1250 mg of calcium and just under 1000 mg of phosphate, making a quart and a half a practical daily target to achieve a favorable balance while limiting meat to perhaps half a pound per day. He also clarified that the body can handle short-term imbalances, but that the ideal has not been precisely defined, and that the safety of very high ratios, such as 2 to 1, depends on the form of the minerals—for example, eggshell and meat versus isolated calcium citrate tablets.

The regulation of this ratio is hormonally integrated and influenced by metabolic context. Peat explained that blood tests for phosphorus are unreliable because its level is maintained by several hormones, with PTH functioning to excrete phosphate. He also noted that a high intake of fructose and good thyroid function support the body's ability to retain magnesium and improve the calcium-to-phosphorus balance, making the exact dietary ratio less critical when sugar intake and metabolic rate are high. This connects the ratio to a broader metabolic picture where adequate thyroid hormone, vitamin D, and magnesium are required for proper calcium regulation, a synergy Peat saw embodied in milk as a near-perfect food.
